Below is a list of the information we may need from you to complete this process.
Documents you may be asked to provide
- Documentation of operation prior to Helene (i.e., 2023 tax returns)
- Business Tax Return
- Receipts of purchased building materials or contractor quotes
- Employer Identification Number (EIN)
- Copies of at least three W-2 forms issued to employees, showing a matching EIN.
- NC Secretary of State ID (SOSID)
